Document by Exception
Documenting a resident's normal daily activities and staff interventions every one or two hours can cause an undue burden for your staff, increasing the likelihood of charting gaps and liability exposures. If your nursing policies state that documentation is to be done by exception only, it is acceptable to limit this type of documentation to occurrences "out of the norm."
If extraordinary circumstances call for more frequent documentation, shift or daily charting is recommended instead of hourly. If hourly documentation can't be avoided, then limit it with a "start" and "end" time/date and flag charts accordingly to increase the likelihood for documentation compliance. If these types of circumstances become frequent or ongoing, you may need to consider transferring the resident to a level of care that provides more acute supervision. Please review your policies and make corrections as needed.
